ALL DOGS OVER 5 MONTHS OF AGE MUST BE LICENSED.
Wisconsin state law requires all dogs to be vaccinated for rabies before a license will be issued.
Please download this form and return with proof of rabies vaccination. A late fee of $5.00 will be charged after April 1, 2025.
